A non-refundable fee of $50 must be submitted with each application and the following required documents.

• Updated Immunization Records (All Students)
  • In accordance with Diocesan Policy, schools in the Catholic Diocese of Salt Lake City do not accept immunization exemptions except for qualified medical exemptions with a letter signed by the student's pediatrician.
• State Issued Birth Certificate
• Most Recent Report Card (grades 1-8)
• Baptismal Certificate (Catholic Only), if applicable
• Educational/Diagnostic/Evaluative Testing Results/IEP/504, if applicable
• Custodial Documentation, if applicable
• STAR Assessments, if transferring from another Utah Catholic School
